This request form is for people dealing with a recurring or unresolved musculoskeletal problem, including:

• Recurring pain, tightness, or stiffness
• Limited movement or restricted range of motion
• Numbness, tingling, or nerve-like symptoms
• Symptoms that keep returning after stretching, massage, chiropractic care, physical therapy, injections, exercise, or other care

Many patients are trying to get back to work, sleep, exercise, golf, pickleball, lifting, walking, sitting, or daily activity with fewer symptoms.

We review your information before recommending the next step. The next step may be a consultation, a comprehensive examination, or a recommendation that another type of care may be more appropriate.
